Acute Occupational Therapist
Job description
Positive Healthcare are recruiting for a Band 6 Occupational Therapist to start work ASAP until the end of February with the high possibility of an extension.
Skills required:
- HCPC registered OT
- Acute hospital experience at band 6 level
- Worked as band 5 OT in variety of rotations, acute, community and mental health if possible
- Previous working experience in the NHS
- Confident in discharge planning in acute and specialist medicine wards
- Effective communication with MDT
- To able to input into frailty pathways as well other relevant caseloads
- Complete generic physical skills assessments such as stairs assessment
- Completion of the Comprehensive Geriatric Assessment (CGA) as appropriate
- Completion of advance cognitive screening tools, access visits and discharge home visits, and prescribe OT equipment
- Addressing ADL needs by identification of care needs and equipment
- Referring patients to Social Services, Community Independence Service referrals and liaison to prevent admission to the downstream wards
